Call me crazy or stupid but day 15 and 16 after my whipple surgery Out on the motorcycle for some much needed wind therapy.  Next up finding out what my choices are for chemo.

OK bad news on my last visit. I was told all clear margins after the surgery. Not so  Still cancer remaining outside the margins near the portal vein. It's to close for radiation or more surgery. So next up is my only choice for chemo Also it's stage III not stage II Adjuvant chemotherapy is indicated. Because of his significant neuropathy we will need to avoid oxaliplatin and Abraxane. After much discussion we have elected to treat with gemcitabine plus capecitabine. Typically this is given 3 weeks on 1 week off. However, because of the patient's performance status and multiple comorbidities I would recommend treatment with 1 week on 1 week off as tolerated. We will restage with CT scans and the patient will return in 2 weeks. We will then discuss details of his chemotherapy. Currently he lives close to 2 hours from our facility. If possible we will arrange for him to be treated closer to home.

Getting my second surgery this year to take more out of my bladder. This time they are going to do a 6 week course of immunotherapy using the TB vaccine to stimulate my immune response. I had never heard of it until now, but it has an 85% efficacy rate to slow or stop it’s return. Cancer sucks bro.

Getting old is not for puscators.
Prescription drugs are a racket in the first degree.
Anyone dealing with them is most likely in "the donut hole."
Tier three drug costs are an example of drug companies mugging.
It is like beating up a person in a wheelchair.
It is a bitch to be taken advantage of when you are vulnerable.

Drug companies spend years...like a decade or more in R&D costs, clinical trials etc. to bring a new drug/product to market. They are entitled to recoup that investment and gamble they took to even try to bring those drugs to market. They are entitled to make a nice profit for that effort. We all want safe drugs, but somewhere in there the FDC is culpable as to the cost of new drugs with the cumbersome rules they enforce on the drug companies.

I understand the cost of R&D etc.
That is not the point. The point is price fixing. They give these drugs away with coupons if you have commercial insurance. However any Medicare Advantage Health Care like UHC you are ineligible.
In addition Medicare sets a gate that if you exceed (Out of Pocket) of $805 for three months you are expected to pay 25% instead of 10%.
A $50 drug for 30 days becomes $150.
There is no oversight on how the costs are derived. The drug companies and the insurance companies decide the price. Medicare sets a $5030 limit per year. That number is not out of pocket. It is what the drug companies charge the insurance companies.
It is easy to exceed that number with tier three drugs.
Once there you pay 25% until you reach $8k for the calendar year of true out of pocket before you are fully covered at 100%.
It is a shell game of price fixing.
No matter what you are paying from 10% to 25% the drug companies are always getting their price. It is a designed price gap ('donut hole'). Set by Medicare, drug companies and health insurance companies.
They know precisely how many pills they sell and will sell. Once a drug is brought to market the prices drop continually to produce that drug.
It is no different than oil. The price per barrel is raised during summer driving because they can do it. It has no effect on their costs.

What makes it so clear is that many other nations pay much less for the same prescription drugs as we do. There are no drug companies selling prescription drugs to other countries at a loss. and actually, Europe and Canada have regulations to assure safety and Europe has even banned prescription drugs that are legal in the U.S because their regulatory boards have deemed them unsafe.
